Ill не имеет форм степеней сравнения, поэтому русское ему стало хуже передается сравнительной формой bad - worse: he was getting worse and worse ему становилось все хуже и хуже. (2.) Sick относится к общему состоянию человека и обычно используется в функции определения. Предикативное употребление sick в значении "болен" является нормой для американского, но не британского варианта.
